Please read carefully before completing registration forms

Registration forms are due by May 10, 2012

 

Early registration is recommended, as we have a limit to our enrollment.  Housing will be assigned in order of registrations received.

 

There are 3 pages of registration forms:

1) Registration Form     

2) Fees Form                

3) Recital Information Form

 

Please print out and complete one registration form (page 1) for each student.  

Fill out a separate form for siblings or parents who are registering for an elective.

 

Please complete one Fees form (page 2) for your family or teen student.

 

If you wish to perform on an afternoon solo recital, complete a Recital Information Form (page 3) and send (along with any necessary piano accompaniment) by June 1.  Students who do not mail the form by this date will not be scheduled for a recital. 

 

Registration forms are due by May 10 for Suzuki Camp and  April 15 for Advanced Chamber Music Program.  After the deadline, please email the Director to inquire about space before mailing your registration.  If a late registration is accepted, a $40 late fee will be added.

 

Please include the $60 registration fee or full payment with your registration.  If you only pay the registration fee, your balance must be paid in full by May 15.

 

You will be notified by email when your registration is received and processed.

 

All tuition, lodging & meal fees may be refunded until May 10 (the registration fee is non-refundable).  Between May 10 and June 10, only lodging and meal payments will be refunded.  After June 10 there are no refunds.
